About Permanent Supportive Housing (PSH)
Community for Positive Aging (CfPA) provides on-site support services to residents in Permanent Supportive Housing communities, helping individuals who have experienced chronic homelessness and face ongoing barriers to stability. Our PSH services are currently offered at Alder House Apartments.
Our PSH team works in partnership with property managers and service providers to build trusting relationships with residents, many of whom live with disabilities, serious health conditions, or behavioral health challenges. Through individualized case management, connection to resources, and consistent presence, we support residents in maintaining housing, improving wellness, and building stronger social connections.
At CfPA, we believe that stable housing is a foundation for a better life—and everyone deserves the opportunity to age in place with dignity, safety, and community.
