Bi-lingual Community Health Worker
Join the Community for Positive Aging (CfPA) and be a key part of building resilient, healthy, and connected communities. As a Bi-lingual Community Health Worker, you will work alongside the Community Health Team and other program staff to implement innovative health initiatives, including Oregon Health Authority Public Health Equity Grants. You’ll lead programs that strengthen well-being, independence, and emergency preparedness for older adults, particularly those who are low-income, BIPOC, LGBTQ+, or marginalized.
This hands-on role allows you to engage directly with participants, lead community programming, and connect residents to resources that promote health, safety, and resilience. By fostering trust, collaboration, and empowerment, you will help communities adapt, respond, and thrive during both everyday challenges and emergencies.

Location: Shared office space in NE Portland at Senior Community Center; regular fieldwork throughout Multnomah, Clackamas, and Washington Counties
Status: Full time- hourly

Key Responsibilities
Collaborate with the Community Health Team and other CfPA program staff to deliver integrated, holistic services across wellness, housing, volunteer, and case management programs.
Lead peer wellness circles, educational workshops, and vaccination clinics, improving immunization awareness and health confidence.
Develop multilingual educational materials and videos for tobacco prevention, health education, and emergency preparedness.
Provide 1:1 navigation and support to help older adults overcome barriers such as language, access, mistrust, and isolation.
Promote healthy and safe environments, including smoke-free housing, access to food resources, and emergency readiness.
Engage residents with resources addressing social determinants of health, such as food insecurity, housing instability, stress, and social isolation.
Incorporate emergency preparedness into community programs, educating older adults on emergency response, safety procedures, and continuity of services during crises.
Serve as an Agency Ambassador, modeling trauma-informed, resident-centered, and strength-based approaches.
Qualifications
Bi-lingual with the ability to communicate effectively across diverse communities.
Community Health Worker Certificate or willingness to obtain, demonstrating commitment to public health and community engagement.
Experience in program or event planning and coordination (minimum 1–2 years), with a track record of managing logistics, volunteers, or community-based initiatives.
Proven experience working with diverse, high-risk, or vulnerable populations, including older adults, BIPOC, LGBTQ+, and marginalized communities.
Strong knowledge of public and community health, social determinants of health, and local resources to connect participants to services and support.
Exceptional interpersonal, communication, and collaboration skills, with the ability to build trust, engage participants, and work effectively across teams and community partners.
Leadership and problem-solving abilities, with experience navigating complex situations, responding to challenges, and fostering community resilience.
Valid driver’s license, proof of insurance, and reliable transportation, with willingness to travel across Multnomah, Clackamas, and Washington Counties.
Adaptable, self-directed, and committed to continuous learning, with the ability to thrive in fast-paced, dynamic environments.
