We are seeking a highly flexible, emotionally intelligent, and experienced Case Manager to join our team supporting residents in our Permanent Supportive Housing (PSH) program. This position is ideal for someone who thrives in dynamic environments, can calmly navigate challenges, and has a deep understanding of housing systems.
You’ll manage a caseload of 15, build trusting relationships with residents ( adults and older adults) and help them maintain stable housing, improve well-being, and foster greater independence all while working alongside a collaborative and committed team.
About Permanent Supportive Housing (PSH)
PSH offers long-term, subsidized housing with voluntary, on-site case management, peer support, and community-building activities. This program serves individuals with a history of chronic homelessness, very low incomes, and one or more disabling conditions. Participants are referred through the Multnomah County Coordinated Access system and services are provided in partnership with the Joint Office of Homeless Services (JOHS).

Case Management & Participant Support
Carry a direct caseload of approximately 15 residents.
Complete screenings, intakes, move-in support, and ongoing assessments.
Develop and update Individual Support Plans (ISPs) based on resident goals.
Hold weekly one-on-one meetings with each participant.
Provide coaching, advocacy, crisis de-escalation, and navigation support.
Connect residents to community resources including benefits, healthcare, behavioral health, and more.
Maintain timely and accurate documentation using HMIS and internal systems.
Collaboration & Community Engagement
Coordinate with internal teams (Resident Services, Traditional Health Workers, Interns & Volunteers) to provide wraparound support.
Build partnerships with external service providers to enhance housing stability and well-being.
Participate in team meetings, community events, and ongoing trainings.
Support data tracking, outreach, and occasional contributions to program communications.

What You Bring
Required Core Competencies
Emotional regulation and flexibility in fast-paced or unpredictable environments.
Experience working in supportive housing, homelessness services, or behavioral health settings.
Familiarity with HMIS and ability to manage accurate, timely documentation.
Ability to de-escalate crises, maintain boundaries, and promote autonomy.
Strong interpersonal skills with an understanding of cultural humility, including respectful engagement with BIPOC, LGBTQ+, disabled, and low-income communities.
Self-directed and highly organized, with excellent communication and follow-through.
Ideal Candidate
1+ year providing case management or similar services to vulnerable populations.
Bachelor’s degree in Social Work, Public Health, or a related field (or equivalent lived/work experience).
Experience with housing navigation, Coordinated Entry, or JOHS-funded programs.
Proficiency with Google Workspace and case management platforms.
Bilingual or multilingual skills (preferred).
Valid driver’s license, proof of insurance, and reliable transportation.
