Permanent Supportive Housing Case Manager – Alder House Apartments
Pay: $27.00 – $28.00 per hour | Full-Time
About the Role
We are hiring a Permanent Supportive Housing (PSH) Case Manager to join our team at Alder House Apartments.
This role is for someone who brings steadiness, professionalism, and compassion to complex, fast-moving work. You will support adults and older adults with histories of chronic homelessness, very low income, and disabling conditions as they maintain housing stability and work toward personal goals.
We are looking for someone who can build trust over time, maintain clear boundaries, and consistently show up for residents and the team.
Permanent Supportive Housing provides long-term housing paired with voluntary, on-site case management, peer support, and community connection. Services are grounded in harm reduction, trauma-informed care, and participant choice.
Residents are referred through Multnomah County Coordinated Access and supported in partnership with the Joint Office of Homeless Services (JOHS).
What You’ll Do
Case Management & Resident Support
Carry a caseload of approximately 15 residents
Complete intakes, assessments, move-in coordination, and ongoing service planning
Develop and update Individual Service Plans (ISPs) based on resident goals
Hold consistent weekly or biweekly one-on-one meetings with participants
Provide coaching, advocacy, crisis de-escalation, and housing stability support
Connect residents to healthcare, behavioral health, benefits, and community resources
Maintain timely, accurate documentation in HMIS and internal systems
Team Collaboration & Reliability
Participate consistently in scheduled team meetings, supervision, and trainings
Coordinate closely with internal staff (Resident Services, THWs, interns, volunteers)
Communicate proactively about caseload concerns, risks, or barriers
Follow through on assigned tasks and shared responsibilities in a timely manner
Contribute to a culture of mutual accountability and team support
Community Engagement & Program Support
Build and maintain relationships with external service providers and partners
Participate in community-building activities and resident events
Support outreach, data tracking, and occasional program communication efforts

Experience de-escalating crises and supporting participant autonomy

Ability to work collaboratively within a team-based service model
Preferred Experience
1+ year of case management, housing services, behavioral health, or similar direct service experience
Experience in Permanent Supportive Housing, Coordinated Entry, or JOHS-funded programs
Bachelor’s degree in Social Work, Public Health, Human Services, or related field (or equivalent lived/professional experience)
Familiarity with HMIS or similar case management systems
Proficiency with Google Workspace
Bilingual or multilingual skills strongly preferred
Valid driver’s license, insurance, and reliable transportation
