Overview
Join the Community for Positive Aging (CfPA) and be a key part of building resilient, healthy, and connected communities. As a Bi-lingual Community Health Worker, you will work alongside the Community Health Team and other program staff to implement innovative health initiatives, including Oregon Health Authority Public Health Equity Grants. You’ll lead programs that strengthen well-being, independence, and emergency preparedness for older adults, particularly those who are low-income, BIPOC, LGBTQ+, or marginalized.
This hands-on role allows you to engage directly with participants, lead community programming, and connect residents to resources that promote health, safety, and resilience. By fostering trust, collaboration, and empowerment, you will help communities adapt, respond, and thrive during both everyday challenges and emergencies.

This includes helping participants access CfPA’s broader services, supporting community outreach and events, and responding to emerging needs in coordination with colleagues.
Key Responsibilities
Community Health and Wellness
Collaborate with the Community Health Team and other CfPA program staff to deliver integrated, holistic services across wellness, housing, volunteer, and case management programs.
Lead peer wellness circles, educational workshops, and vaccination clinics, improving immunization awareness and health confidence.
Develop multilingual educational materials and videos for tobacco prevention, health education, and emergency preparedness.
Provide 1:1 navigation and support to help older adults overcome barriers such as language, access, mistrust, and isolation.
Promote healthy and safe environments, including smoke-free housing, access to food resources, and emergency readiness.
Engage residents with resources addressing social determinants of health, such as food insecurity, housing instability, stress, and social isolation.
Incorporate emergency preparedness into community programs, educating older adults on emergency response, safety procedures, and continuity of services during crises.
Identify emerging participant and community needs and communicate relevant observations and feedback to colleagues to help strengthen services and inform programming.
Cross-Program Collaboration and Organizational Support
Work collaboratively with colleagues across CfPA programs to support coordinated, person-centered services and connect older adults with relevant programs, resources, and community partners.
Assist with organizational activities, including community events, outreach, educational programming, and participant engagement, as appropriate to the needs of the organization and the scope of the role.
Coordinate with colleagues to facilitate referrals, share relevant information appropriately, and help participants navigate CfPA’s services and community resources.
Provide support for shared program and organizational priorities when needs arise, working with supervisors and colleagues to clarify responsibilities and balance competing priorities.
Contribute to a welcoming, inclusive, and supportive environment for participants, volunteers, colleagues, and community partners.
Community Engagement and Representation
Build and maintain relationships with community organizations, service providers, and other partners to strengthen access to health resources and support for older adults.
Represent CfPA and its Community Health Team at community events, outreach activities, educational opportunities, and partner meetings, as appropriate.
Support community education and engagement initiatives that raise awareness of CfPA programs and promote health equity, community resilience, and social connection.
Trauma-Informed and Person-Centered Practice
Serve as an agency ambassador, modeling trauma-informed, resident-centered, and strengths-based approaches.
Interact with participants and colleagues with compassion, cultural humility, and respect for individual dignity, autonomy, and lived experience.
Maintain appropriate professional boundaries and protect confidential participant information in accordance with CfPA policies and applicable requirements.
Qualifications
Bi-lingual with the ability to communicate effectively across diverse communities.
Community Health Worker Certificate or willingness to obtain, demonstrating commitment to public health and community engagement.
Experience in program or event planning and coordination (minimum 1–2 years), with a track record of managing logistics, volunteers, or community-based initiatives.
Proven experience working with diverse, high-risk, or vulnerable populations, including older adults, BIPOC, LGBTQ+, and marginalized communities.
Strong knowledge of public and community health, social determinants of health, and local resources to connect participants to services and support.
Exceptional interpersonal, communication, and collaboration skills, with the ability to build trust, engage participants, and work effectively across teams and community partners.
Leadership and problem-solving abilities, with experience navigating complex situations, responding to challenges, and fostering community resilience.
Ability to work collaboratively across programs, recognize shared organizational responsibilities, and contribute to team priorities while maintaining accountability for core job responsibilities.
Flexibility, initiative, sound judgment, and the ability to adapt to changing community and organizational needs while communicating effectively about priorities and capacity.
Valid driver’s license, proof of insurance, and reliable transportation, with willingness to travel across Multnomah, Clackamas, and Washington Counties.
Adaptable, self-directed, and committed to continuous learning, with the ability to thrive in fast-paced, dynamic environments.
